Researchers at Johns Hopkins University and Duke University have created a powerful new AI tool that could change how we predict, track, and manage infectious diseases like COVID-19, flu, and RSV.
The tool is called PandemicLLM, and it uses large language models (LLMs)—the same kind of technology behind ChatGPT—to think and reason like a public health expert. Instead of just crunching numbers, it understands real-world conditions like new virus variants, policy changes, and vaccination rates to forecast what might happen next.
This new research is published in Nature Computational Science (2025).
🔍 Why Was PandemicLLM Created?
During the COVID-19 pandemic, traditional models had trouble predicting disease spread when situations were changing quickly, such as when a new variant appeared or lockdown rules were updated.
“When conditions were stable, models worked okay,” said Dr. Lauren Gardner, a Johns Hopkins expert who created the famous COVID-19 dashboard. “But during sudden changes, we couldn’t keep up. This new tool fills that gap.”
🧠 How Does PandemicLLM Work?
PandemicLLM uses four types of data to make smarter predictions:
- State-level data – Includes demographics, healthcare systems, and political factors
- Epidemiological data – Case counts, hospitalizations, vaccine rates
- Public health policies – Types and strictness of government regulations
- Genomic data – New virus variants and how fast they’re spreading
By analyzing all this information in real-time, PandemicLLM predicts infection surges and hospitalizations up to three weeks in advance, outperforming the best models used by the CDC’s COVIDHub.

What makes PandemicLLM special is its ability to reason through constantly changing situations, just like a human would. It’s especially effective when an outbreak is in flux and past data alone isn’t enough.
“We usually use the past to guess the future,” said Dr. Hao “Frank” Yang, an assistant professor at Johns Hopkins. “This AI model uses real-time information, which makes it much smarter.”
The tool can be adapted for any infectious disease, including monkeypox, bird flu, and RSV, making it a valuable tool for future pandemics.
