“Rudra on Rails: Putting Medical Care on Track for Remote Railway Workers”

Healthcare access in remote India has always been a challenge. But now, an innovative project from the Central Railway’s Bhusawal division is showing how creativity can bring care closer to those who need it most.

An abandoned AC three-tier coach has been repurposed into India’s first railway hospital coach, named Rudra. Designed to serve railway employees and their families in far-flung stations, Rudra brings doctors, diagnostics, and even telemedicine right to the tracks.

The Birth of Rudra

Divisional Railway Manager Ity Pandey identified a huge gap: railway staff posted in remote areas often had little or no access to timely medical services. Instead of waiting for funds, the divisional team used its own hospital budget to convert a discarded coach from the Matunga depot into a fully functional clinic.

Facilities on Board

The coach has been carefully redesigned to include:

  • Consulting rooms for physicians and specialists
  • ECG and blood collection unit
  • Gynaecology examination area
  • Dental chair and minor procedure space
  • Telemedicine links to connect with specialists in bigger cities
  • Vaccine storage and sterilisation units

Patients also receive unique IDs, ensuring continuity of care with divisional hospitals.

Services and Camps

  • Rudra hosts fortnightly medical camps across railway stations in nine districts.
  • Doctors from multiple specialties—general medicine, pediatrics, surgery, orthopedics, gynecology, and dentistry—rotate to serve.
  • The first camp on 18 January 2025 treated 259 patients; by March, Rudra had already reached 1,266 consultations.
  • Medicines are dispensed on-site, and referrals are made when higher care is required.

Expansion Plans

A second coach is already in progress, focusing on ophthalmology and dental services, with space for a small operating theater to perform cataract surgeries and minor dental procedures.

If outcomes remain strong, the model may be replicated across other railway divisions in India.
